Clay County's Community Health Improvement Plan (CHIP) is a collaborative effort uniting various stakeholders to enhance our community's health. Our objectives, shaped by the Community Health Assessment, focus on key health issues for the next five years. We invite you to engage with us as we review our progress from the past year and hear updates from community partners vital health areas affecting our residents.
